Understanding Cirrhosis and How It Affects the Skin

Cirrhosis is a serious liver disease where healthy liver tissue gradually gets replaced by scar tissue. This scarring happens over months or years and makes it harder for the liver to work properly. The liver is one of the largest organs in your body, and it does many important jobs. It filters your blood, helps digest food, stores energy, and removes harmful substances. When cirrhosis develops, the liver cannot perform these functions as well, and this affects many systems in your body—including your skin.

The skin problems that develop with cirrhosis are not just cosmetic concerns. They are signs that your liver is struggling. About 30 to 50 percent of people with cirrhosis develop visible skin changes. These changes happen because a damaged liver cannot regulate certain hormones and cannot properly process substances that affect blood flow and skin appearance. Understanding these skin signs can help you recognize when to talk to a doctor about your liver health.

Cirrhosis can develop from several causes. Chronic hepatitis C infection accounts for roughly 27 percent of cirrhosis cases in the United States. Alcohol-related liver disease causes about 21 percent of cases. Other causes include hepatitis B, fatty liver disease, autoimmune liver disease, and bile duct problems. No matter the cause, once cirrhosis develops, the skin often shows visible warning signs. Some of these signs appear early, while others develop as the liver damage becomes more severe.

The connection between your liver and your skin happens through your bloodstream and hormone levels. Your liver normally breaks down hormones like estrogen. When cirrhosis develops, hormones build up in your blood because the damaged liver cannot process them. High hormone levels change blood vessel behavior and skin pigmentation. Additionally, cirrhosis affects how your body handles proteins and how blood flows through vessels, all of which show up on your skin.

Practical takeaway: If you notice unusual skin changes developing over weeks or months, especially rashes, discoloration, or other symptoms described in this guide, mention them to your doctor. These changes may signal liver problems that need medical attention.

Spider Angiomas: Recognizing These Distinctive Red Marks

Spider angiomas are among the most common skin signs of cirrhosis. These are small, red, spider-shaped marks that appear on the skin. A spider angioma has a central red dot with thin red lines radiating outward, resembling a tiny spider. They typically measure 2 to 10 millimeters across. Most commonly, they appear on the chest, face, neck, and hands, though they can develop anywhere on the body. Studies show that spider angiomas appear in about 33 percent of people with cirrhosis.

Spider angiomas form because of high estrogen levels in the blood. When the liver is healthy, it breaks down estrogen from your bloodstream. A cirrhotic liver cannot do this efficiently, so estrogen accumulates. High estrogen causes tiny blood vessels called arterioles to dilate—meaning they expand and stay enlarged. This expansion creates the characteristic spider shape you can see on the skin.

You can identify a spider angioma by looking for these features: a central red spot with radiating lines, blanching when you press on it (the red fades when you apply pressure, then returns when you release), and a location on the upper body. A single spider angioma might not be concerning, but the presence of multiple spider angiomas often suggests cirrhosis or advanced liver disease. Some healthy people develop one or two spider angiomas naturally, but five or more typically indicates liver disease.

Spider angiomas tend to appear and disappear unpredictably. Sometimes they fade, and sometimes new ones develop. If your liver function improves—for example, if you stop drinking alcohol or receive treatment for hepatitis—spider angiomas may gradually disappear. Conversely, if liver damage worsens, more spider angiomas usually develop. They do not cause pain or itch, but their presence is a visible signal that something is wrong with your liver.

Some people worry about removing spider angiomas for cosmetic reasons. Dermatologists can remove them with laser treatment or electrocautery, but they may return if the underlying liver disease continues. The medical priority is treating the liver disease itself, not removing the cosmetic marks. If you notice spider angiomas appearing, this is a sign to seek medical evaluation of your liver function.

Practical takeaway: Use the blanching test to check suspicious red marks. Press firmly on a red spot on your chest or arms. If it fades and returns when you release pressure, it may be a spider angioma. If you find several of these marks, discuss them with your doctor as soon as reasonably possible.

Palmar Erythema: Understanding Red Palms and What They Mean

Palmar erythema refers to redness and heat in the palms of your hands. The palms become abnormally red, and they often feel warm or hot. The redness typically affects the fleshy areas at the base of your thumb and fingers (the thenar and hypothenar eminences) and may also spread across the entire palm. About 23 percent of people with cirrhosis experience palmar erythema. Interestingly, the back of the hands typically appears normal or less red than the palms.

Like spider angiomas, palmar erythema develops because of high estrogen and other hormone imbalances related to cirrhosis. The redness results from increased blood flow to the palms due to dilated blood vessels. People with palmar erythema often describe their palms as feeling warm or burning. Some people notice they have trouble keeping their hands cool, or that their palm temperature fluctuates.

You can check yourself for palmar erythema by looking at your palms under good lighting and comparing them to someone else's hands. Normal palms have some color variation, but they do not appear bright red or blotchy. With palmar erythema, you will notice distinct redness that may come and go depending on your activity level, temperature, and stress. The redness often increases when you hold your hands above your head or apply pressure to your palms.

Palmar erythema can develop with conditions other than cirrhosis, including pregnancy, rheumatoid arthritis, and hyperthyroidism. However, when combined with other signs of liver disease—such as spider angiomas, jaundice, or abdominal swelling—palmar erythema strongly suggests cirrhosis. Your doctor will consider all your symptoms and run blood tests and imaging studies to determine whether your palms are red due to liver disease or another cause.

The good news is that palmar erythema is reversible if liver function improves. People who stop drinking alcohol and whose liver damage stabilizes may see their palmar erythema fade over months. This makes the palms a useful indicator for tracking whether your liver is improving or deteriorating over time.

Practical takeaway: Check your palms regularly for unusual redness, especially in the fleshy areas at the base of your fingers and thumb. Note whether they feel warm. Combined with other symptoms, this observation can help your doctor assess your liver health.

Jaundice, Skin Discoloration, and Other Color Changes

Jaundice is a yellowing of the skin and whites of the eyes that occurs when bilirubin—a yellow pigment from broken-down red blood cells—builds up in your blood. Normally, your liver processes bilirubin and passes it into bile to be excreted. When cirrhosis damages the liver, bilirubin cannot be processed, so it accumulates in your bloodstream and deposits in your skin. Jaundice typically appears when bilirubin levels exceed 2 to 3 milligrams per deciliter of blood. Studies show that about 50 percent of people with advanced cirrhosis develop jaundice.

Jaundice usually starts in the whites of the eyes (sclera) and gradually spreads to the skin. You might first notice a yellowish tint to the whites of your eyes when looking in a mirror. As bilirubin levels rise, your entire skin takes on a yellow or greenish-yellow tone. Some people describe it as looking like they have a permanent tan. The yellowing tends to be more noticeable on areas that receive little sun exposure, such as your chest and inner arms. The palms and soles of your feet may also turn yellow.
