Understanding Pilonidal Cyst: What It Is and Why Treatment Matters
A pilonidal cyst is a small sac that forms in the skin, typically at the top of the crease between the buttocks. The name comes from Latin words meaning "nest of hair," because these cysts often contain hair and skin debris. The condition affects approximately 0.7% to 0.12% of the population in the United States, though some medical sources suggest the actual number may be higher due to unreported cases.

The cyst develops when hair follicles become irritated or damaged, creating a small cavity in the skin. Over time, loose hairs can enter this cavity and become trapped, along with dead skin cells and other debris. This creates an environment where bacteria can grow, leading to infection and inflammation. Most people don't realize they have a pilonidal cyst until it becomes infected and painful.
Symptoms vary depending on whether the cyst is infected. A non-infected cyst may cause minimal discomfort and might go unnoticed for months or years. An infected cyst typically produces pain, redness, warmth to the touch, and drainage of fluid or pus. Some people experience itching or a discharge from a small opening in the skin. In more severe cases, fever and swelling may develop.
The condition most commonly affects people in their teenage years and early adulthood, typically between ages 15 and 40. Men are affected roughly twice as often as women. Risk factors include family history, excessive body hair, prolonged sitting, poor hygiene, or trauma to the area. Understanding what causes pilonidal cysts helps explain why certain treatments focus on hair removal and keeping the area clean and dry.
Practical Takeaway: Recognizing the early signs of a pilonidal cyst—such as small openings or pits in the skin, drainage, or discomfort in the area between the buttocks—allows you to seek treatment before the condition becomes severely infected. If you notice these symptoms, scheduling an appointment with a healthcare provider is the first step toward exploring your treatment options.
Conservative Treatment Options for Mild Cases
Conservative treatments are typically recommended as the first approach, especially for people experiencing mild symptoms or those whose cysts are not severely infected. These methods focus on managing pain, preventing infection, and reducing the factors that contribute to cyst development.

One of the most important conservative approaches involves keeping the area clean and dry. This means washing the region between the buttocks daily with soap and water, then thoroughly drying it. Some healthcare providers recommend using a hair dryer on a cool setting to ensure complete dryness. Keeping sweat and moisture away from the area reduces the environment where bacteria thrive. People should also avoid prolonged sitting when possible, taking regular breaks to stand and move around.
Hair removal is another key component of conservative treatment. Removing hair from the area—whether through shaving, waxing, or using depilatory creams—reduces the amount of loose hair that can enter the cyst. Many people find success with regular shaving of the entire sacral region (the lower back and buttock area). Some providers recommend depilatory creams, though these require careful application to avoid skin irritation. Laser hair removal is a more long-term option that some people pursue, though it typically requires multiple sessions over several months.
Antibiotics may be prescribed if signs of infection appear, such as increased warmth, redness, drainage, or fever. Common antibiotics used include cephalexin or clindamycin. While antibiotics can reduce the active infection, they typically do not eliminate the cyst itself. Pain management through over-the-counter medications like ibuprofen or acetaminophen can help control discomfort while pursuing other treatments.
For some people, conservative management works well enough to keep symptoms manageable over many years. Studies show that recurrence rates vary, but roughly 40% of people who use conservative treatments experience recurrence within several years. However, this approach carries minimal risk and can be worth attempting before considering more invasive options.
Practical Takeaway: If you have a mild or newly diagnosed pilonidal cyst, start with conservative care: maintain scrupulous hygiene, keep the area dry, and remove hair regularly. Document how these changes affect your symptoms over 2-4 weeks to determine whether conservative treatment is working for you or whether you need to explore additional options.
Minimally Invasive and Drainage Procedures
When conservative treatments fail or when a cyst becomes acutely infected and painful, minimally invasive procedures offer a middle ground between self-care and full surgical removal. These procedures are typically performed in an outpatient setting and allow for faster recovery than traditional surgery.

Needle aspiration or incision and drainage is often the first procedure used for an infected pilonidal cyst. A healthcare provider makes a small incision to allow pus and infected material to drain. This provides immediate pain relief and allows the infection to resolve. However, this approach is temporary—studies show that roughly 50-60% of people who undergo simple drainage experience recurrence within weeks or months. For this reason, drainage alone is typically used as an emergency measure to relieve acute infection rather than as a definitive treatment.
Minimally invasive surgical techniques have become increasingly popular over the past decade. One such technique is endoscopic ablation, where a thin camera and specialized instruments allow the surgeon to visualize and remove the cyst cavity without making large incisions. Another approach involves using a laser or radiofrequency device to ablate (burn away) the cavity lining. These techniques result in smaller wounds, less pain, and faster healing compared to traditional open surgery. Recovery typically takes 1-2 weeks rather than the 3-4 weeks required for open procedures.
The pit picking procedure, or minimally invasive pit excision, focuses on identifying and removing the small openings (pits) where hair enters the cyst. A surgeon makes tiny incisions and removes the tissue around these pits. This approach has gained attention because it specifically targets the problem points rather than removing the entire cyst cavity. Studies report recurrence rates ranging from 10-25%, which is significantly lower than simple drainage.
Another option is the use of fibrin glue or other tissue sealants injected into the cyst cavity to promote healing and prevent recurrence. This technique may be combined with other procedures. Some healthcare providers use a technique called "marsupialization," where the cyst is partially opened but left to heal from the inside out, reducing the wound size compared to full excision.
Practical Takeaway: If your pilonidal cyst has not responded to conservative care or has become acutely infected, ask your healthcare provider about minimally invasive options. These procedures typically offer better recurrence rates than simple drainage while allowing for faster recovery than traditional surgery. Discuss the specific techniques available at your local medical facility and the recurrence rates associated with each.
Surgical Removal and Advanced Techniques
When pilonidal cysts are large, severely infected, or have recurred multiple times after other treatments, surgical removal may become necessary. Traditional open surgery involves removing the entire cyst cavity, though the method of closure and wound management significantly affects healing time and recurrence rates.

In the primary closure technique, the surgeon removes the cyst cavity and then closes the wound with stitches. This approach allows for faster healing—typically 3-4 weeks—since the wound is sealed. However, if the wound becomes infected or opens before healing is complete, recovery can be significantly delayed. Studies indicate that primary closure has recurrence rates of approximately 15-30%, partly because complete closure of the deep wound cavity can be difficult to achieve.
The open healing technique leaves the wound open after cyst removal, allowing it to heal from the inside out. This process takes 6-12 weeks and requires ongoing wound care, including regular cleaning and dressing changes. While the extended healing time is a significant drawback, this method has lower recurrence rates—typically 5-15%—because there is no sealed cavity where infection or recurrence can develop undetected. People who choose this approach must be committed to consistent wound care during the healing period.
Flap procedures represent an advanced approach where tissue from surrounding areas is used to fill the defect left by cyst removal. The Karydakis procedure moves the surgical scar away from the midline (center) of the buttocks, reducing friction and moisture in that area. The Limberg flap, or Cleft Lift procedure, removes the cyst and reshapes the skin to flatten or reduce the depth
