What Frontotemporal Dementia Is and How It Differs From Other Dementias
Frontotemporal dementia (FTD) is a group of brain disorders that damage the front and side parts of the brain. These areas control personality, behavior, language, and decision-making. FTD is sometimes called frontotemporal lobar degeneration or Pick's disease, depending on which type someone has. Unlike Alzheimer's disease, which typically starts with memory problems, FTD often begins with changes in personality and behavior.

FTD accounts for 5 to 10 percent of all dementia cases, making it less common than Alzheimer's disease but more common than people realize. It typically strikes people between ages 40 and 70, though it can occur at younger or older ages. About 60 percent of people diagnosed with FTD are younger than 65. This makes FTD particularly challenging because it often affects people during their working years and active family life.
The brain damage in FTD happens in specific regions. The frontal lobe controls impulse control, planning, and social behavior. The temporal lobe handles language and emotional understanding. When neurons in these areas die and shrink, the brain cannot perform these functions properly. This is different from Alzheimer's, where memory loss typically appears first because the hippocampus is affected earliest.
There are three main types of FTD. Behavioral variant FTD affects personality and behavior first. Primary progressive aphasia affects language and speech first. Semantic variant primary progressive aphasia affects word meaning and understanding. A person might have one type or features of multiple types. This variation makes FTD diagnosis more complicated than some other dementias.
Practical takeaway: Understanding that FTD primarily affects the front and side brain regions helps explain why personality and behavior changes appear before memory loss. Family members often report that their relative "is not themselves anymore" rather than forgetting things, which is a key difference from other dementias.
Early Behavioral and Personality Changes to Watch For
The earliest signs of FTD often involve noticeable shifts in how someone acts and relates to others. A person who was once careful with money might suddenly spend recklessly. Someone known for kindness might become irritable or emotionally distant. These changes typically appear gradually over weeks or months but can feel sudden to family members who see them daily. The person themselves often does not notice these changes, which makes FTD particularly difficult for loved ones.

Common behavioral changes include lack of empathy or concern for others, loss of social awareness, reduced inhibition, poor judgment, and compulsive behaviors. A person might say inappropriate things without realizing how offensive they sound. They might neglect personal hygiene or appearance despite previously caring about how they looked. Some people become obsessed with collecting items, watching the same show repeatedly, or following rigid routines. Others become apathetic and lose interest in activities they once enjoyed.
Impulse control problems are particularly notable in FTD. Someone might act on thoughts without considering consequences. This could mean aggressive outbursts, risky financial decisions, or inappropriate sexual behavior. These actions are not intentional or malicious—they reflect damage to the brain regions that normally prevent impulsive reactions. Family members often misinterpret these behaviors as character flaws or willful misconduct, not realizing they stem from brain disease.
Food-related behaviors sometimes emerge in FTD, particularly a condition called hyperorality. A person might eat non-food items, overeat dramatically, or become fixated on specific foods. One documented case involved a man who began eating entire sticks of butter straight from the refrigerator, a behavior completely foreign to his previous habits. Compulsive behaviors might include repetitive movements, tapping, or organizing objects in particular ways.
Practical takeaway: Keep a journal noting when personality or behavioral changes occur and what triggers them. Share specific examples with doctors using concrete instances rather than general statements. For example, "He yelled at his sister three times this week for things that previously would not have bothered him" provides more useful information than "He is more irritable."
Language and Communication Problems Associated With FTD
Language difficulties in FTD vary depending on which type someone has, but all forms can affect communication. In primary progressive aphasia, language problems appear first and most noticeably. A person might struggle to find words, repeat phrases without meaning, or lose the ability to understand what others say. Early on, the person often recognizes they are having trouble, which can be frustrating and embarrassing.

One pattern seen in FTD is anomia, or difficulty retrieving specific words. A person knows what they want to say but cannot access the word. They might describe the object instead—"the thing you write with" instead of "pen"—or become stuck mid-sentence. This differs from Alzheimer's memory loss because the person is not forgetting what words mean; rather, they cannot retrieve them from their brain's storage.
Speech can become repetitive in FTD. A person might repeat the same phrase over and over, even when they do not realize they already said it minutes earlier. Some people develop what is called palilalia, where they repeat words or phrases with increasing frequency. Others might use the same sentence structure repeatedly or get stuck on particular words. These patterns are involuntary and reflect how the damaged brain is processing language.
Comprehension difficulties also emerge, though sometimes gradually. A person might understand short, simple statements but struggle with complex sentences. They might lose the thread of a conversation or misinterpret what someone means. In semantic variant FTD, a person loses understanding of what words mean rather than the ability to speak them. They might see a cat and not recognize it as an animal or know what to do with a fork, even though they can speak fluently.
Practical takeaway: When communicating with someone experiencing language changes, speak slowly using short sentences, give them time to respond, and avoid correcting speech errors. Written notes or pictures can supplement spoken words. Keep conversations in quiet environments with fewer distractions to reduce confusion and frustration.
Physical and Motor Symptoms That May Develop
While FTD primarily affects behavior and language early on, physical symptoms can develop as the disease progresses. These symptoms vary depending on which brain regions are most affected. Some people develop movement problems similar to Parkinson's disease, including tremors, rigidity, or slowed movement. Others experience weakness, loss of balance, or difficulty coordinating movements.

Apraxia of speech is a physical symptom where the brain cannot properly direct the muscles needed for speech. The person wants to speak and understands what they want to say, but the signal from brain to mouth muscles does not work correctly. This is different from language problems—it is a motor control issue. Speech might become slow, effortful, or distorted. The person might struggle to move their lips or tongue properly even though these muscles are not actually weak.
Some people with FTD develop amyotrophic lateral sclerosis (ALS) symptoms, which involve progressive muscle weakness and paralysis. About 5 to 15 percent of FTD cases include ALS features. In these cases, muscles gradually weaken, making movement increasingly difficult. The person might have difficulty swallowing, which can be dangerous because food or liquid might enter the lungs instead of going to the stomach.
Swallowing problems, called dysphagia, can develop as FTD progresses. A person might cough when drinking, feel like food is stuck, or aspirate (inhale food or liquid into the lungs). This is a serious concern because aspiration can lead to pneumonia. Some people develop such severe swallowing problems that they cannot eat or drink safely by mouth and may eventually require feeding tubes.
Practical takeaway: When someone develops physical symptoms, work with occupational and physical therapists to maintain safety and independence as long as possible. Be alert for swallowing difficulties and watch for signs like coughing during meals, drooling, or complaints that food feels stuck. These warrant evaluation by a speech-language pathologist.
How FTD Progresses and Changes Over Time
Frontotemporal dementia typically progresses over 8 to 10 years from initial symptoms to severe disability, though some people decline faster or slower. The rate of progression varies significantly between individuals, and predicting someone's specific timeline is not possible. Early-stage FTD might last 2 to 4 years, middle-stage 2 to 10 years, and late-stage until
