What Urine Testing Is and Why It's Used
Urine testing, also called urinalysis, is one of the most common medical procedures performed in the United States. According to the American Association for Clinical Chemistry, urinalysis accounts for approximately 5% of all laboratory tests ordered by healthcare providers. This test examines the physical, chemical, and microscopic properties of urine to detect various health conditions.

Urine testing serves multiple purposes in medical care. Healthcare providers order urinalysis to screen for infections, kidney disease, diabetes, liver problems, and metabolic disorders. The test can also detect the presence of certain medications, drugs, or substances in your system. Some employers and organizations require urine testing as part of employment screening or safety programs. The test is non-invasive, relatively inexpensive (typically costing between $25 and $100 without insurance), and provides valuable information about your health status.
The human body produces urine as it filters waste products from the bloodstream through the kidneys. This waste includes urea, creatinine, uric acid, and excess water and electrolytes. When something goes wrong in your body, it often shows up in your urine. For example, if you have a urinary tract infection, bacteria appear in the urine sample. If your blood sugar is very high due to diabetes, glucose spills into the urine and becomes detectable. This is why urine testing has remained a standard diagnostic tool for over a century.
Different situations call for different types of urine tests. A routine urinalysis involves analyzing a single urine sample. A 24-hour urine collection involves collecting all urine produced over an entire day, which provides information about kidney function and certain hormone levels. Some tests require specific preparation or timing. Understanding which type of test is being performed helps you know what to expect and how to prepare.
Practical takeaway: Before any medical appointment where you might provide a urine sample, ask your healthcare provider whether any special preparation is needed. Most routine urinalysis tests require no preparation, but knowing in advance prevents confusion or the need to reschedule.
How to Collect a Urine Sample Correctly
Proper sample collection is critical to getting accurate test results. Contaminated or improperly collected samples can lead to false positive or false negative results, potentially affecting your diagnosis and treatment. Healthcare facilities provide specific instructions for collection to minimize contamination and ensure reliable results.

For a standard urinalysis, most facilities use what's called a "clean-catch" or "midstream" collection method. This involves several steps: First, wash your hands with soap and water. Second, use an antiseptic wipe (usually provided by the facility) to clean the area around your urethra—the opening where urine exits your body. Third, begin urinating into the toilet, then stop the flow and position a sterile collection cup to catch the middle portion of your urine stream. Finally, finish urinating into the toilet. This midstream approach captures urine that's less likely to contain bacteria from your skin, which would skew results.
Women should be particularly careful with the clean-catch method. The Centers for Disease Control and Prevention recommends that women wipe from front to back to prevent contamination from bacteria found in fecal matter. Men should retract the foreskin (if uncircumcised) before cleaning the area and collecting the sample. For either sex, the antiseptic wipe should dry completely before collection to avoid interfering with test results.
If you're unable to collect a sample yourself due to age, mobility limitations, or medical conditions, healthcare staff can assist with the collection process. In hospital settings, samples are sometimes collected through a catheter—a thin tube inserted into the bladder to drain urine directly. Catheterized samples eliminate contamination from skin bacteria entirely, which is why they're used for patients where contamination is a concern.
The collected sample should be stored properly until testing. Most facilities prefer that samples be tested within one to two hours of collection. If storage is necessary, the sample should be refrigerated to slow bacterial growth and prevent changes in the urine's composition. Before testing, laboratory technicians often examine the sample visually, noting its color, clarity, and odor, as these characteristics can indicate certain conditions.
Practical takeaway: When collecting a urine sample, follow all written and verbal instructions provided by your healthcare facility. If you're unsure about any step in the collection process, ask a staff member for clarification before beginning—it takes only a few minutes and ensures your results are accurate.
Understanding the Three Components of Urinalysis
A complete urinalysis consists of three distinct parts: physical examination, chemical analysis, and microscopic examination. Each component provides different information about your health. Understanding what each part measures helps you comprehend what your results mean.

The physical examination is the first step and involves observing the urine's appearance without any equipment. Laboratory technicians evaluate several characteristics. Color ranges from pale yellow to amber, with normal urine typically being pale to light yellow. Dark yellow or amber urine often indicates dehydration. Brown or tea-colored urine might suggest kidney problems or certain liver conditions. Red or pink urine could indicate blood in the urine (hematuria), though certain foods like beets can temporarily cause pink-tinged urine. Cloudiness or turbidity can suggest the presence of bacteria, white blood cells, or crystals. Normal urine is clear. Odor is also noted—normal urine has a slightly pungent smell, but unusual odors like a fruity smell might indicate diabetes complications, or a foul smell might suggest infection.
Chemical analysis uses specially treated dipstick strips that change color when exposed to different substances in urine. These strips test for approximately ten different chemicals in a matter of minutes. The test checks for protein, which shouldn't normally appear in urine—its presence might indicate kidney disease or urinary tract infection. It measures pH (acidity or alkalinity), with normal urine being slightly acidic. It detects glucose, which shouldn't be present unless your blood sugar is very high. The test identifies ketones, which appear when the body is breaking down fat for energy rapidly. It looks for blood, which could indicate infection, kidney stones, or other problems. The test also measures leukocyte esterase (indicating white blood cells) and nitrites (suggesting bacteria), both pointing to possible infection. Specific gravity measures how concentrated your urine is. Bilirubin and urobilinogen indicate liver function. Some facilities also test for ascorbic acid (vitamin C).
Microscopic examination involves looking at urine under a microscope to identify cells, bacteria, crystals, and other structures. Technicians count and identify red blood cells, white blood cells, epithelial cells (skin cells), bacteria, yeast, casts (protein-based tubular structures), and crystals. The number and type of these elements provide additional diagnostic information. For example, finding numerous white blood cells and bacteria confirms a urinary tract infection. Finding red blood cell casts suggests a kidney problem more serious than simple hematuria.
Practical takeaway: When you receive urinalysis results, ask your healthcare provider to explain each abnormal finding. Results aren't automatically alarming—many findings require additional testing or clinical context to interpret. Understanding what was measured helps you ask informed questions about what comes next.
Common Reasons for Abnormal Results and What They May Indicate
Abnormal urinalysis results don't automatically indicate serious disease, but they do warrant further investigation. Many conditions produce detectable changes in urine, and understanding what causes different results helps you work with your healthcare provider on next steps.

Presence of bacteria and white blood cells typically indicates a urinary tract infection (UTI). UTIs are among the most common bacterial infections, affecting approximately 150 million people worldwide annually according to the National Institutes of Health. Symptoms include painful urination, urgency, frequency, and lower abdominal discomfort. However, some people have bacteria in their urine without symptoms. Asymptomatic bacteriuria is common in older adults and pregnant women but doesn't always require treatment unless the person is pregnant or about to undergo urinary procedures.
Protein in urine (proteinuria) shouldn't appear in significant quantities because protein molecules are too large to normally pass through healthy kidneys. Small amounts occasionally appear after strenuous exercise, fever, or stress. Persistent protein can indicate kidney disease, diabetes complications, high blood pressure effects, or lupus. The amount of protein matters—healthcare providers distinguish between trace amounts, small amounts, and large amounts. Sometimes a repeat test is ordered to confirm the finding, since a single abnormal result doesn't automatically confirm
